Short answer: For the first few hours after Botox, stay upright, don't rub or massage the treated area, and skip strenuous exercise, saunas, and lying flat. Light facial movements are encouraged. Avoid alcohol and blood-thinners around your appointment to reduce bruising. Most restrictions lift after 24 hours, the rules are simpler than the internet suggests.
Botox aftercare has collected a lot of myths. The real list of things that matter is short, and most of it applies only to the first few hours while the product settles into the right muscles. Here's the evidence-based version, minus the folklore.
The first few hours
The core idea is to keep the product where your injector placed it. For roughly four hours, stay upright (don't lie flat), and don't rub, massage, or apply pressure to the treated areas, that includes facials, tight hats, or a face-down nap. Skip strenuous exercise, hot yoga, saunas, and anything that spikes blood flow to your face that day. Gentle, natural facial expressions are fine and don't move the product.
Reducing bruising
Bruising is the most common minor side effect and it's mostly about blood-thinning. When your provider approves, avoiding alcohol, aspirin, ibuprofen, fish oil, and vitamin E for a day or two before and after can help. Arnica or a cold compress can reduce a bruise if one appears. Tiny injection bumps disappear within an hour; small bruises fade over a few days and are easily covered with makeup after 24 hours.
After 24 hours
Most restrictions lift the next day, normal exercise, skincare, and sleeping positions are fine. Remember that results build over 3–5 days and peak at about two weeks, so don't judge (or ask for a touch-up) before the two-week mark. If anything looks asymmetric at two weeks, that's the right time to see your injector for a small adjustment.

Botox Aftercare: The First 24 Hours: FAQ
Skip strenuous exercise for the rest of the day, about 24 hours is the common guidance. Light walking is fine; it's the heavy, blood-flow-spiking workouts and inversions to avoid initially.
Stay upright for about four hours afterward, then it's fine. The goal is simply to keep the product settling where it was placed while it takes hold.
Rubbing or massaging the area in the first few hours could theoretically move the product to unintended muscles. After the settling window, normal skincare and touching are fine.
The first-few-hours rules help ensure the product stays where intended, and avoiding blood-thinners reduces bruising. Beyond that, most 'aftercare' advice online is myth, the biggest factor in your result is the injection itself.
