Filler Swelling Stages: Day by Day

Short answer: Filler swelling peaks in the first 24–72 hours, then eases over a week; the final result settles by about two weeks (sometimes up to four for under-eyes). Lips look biggest on days 1–3. Sudden, painful, one-sided, or blanching swelling, especially with skin color changes, is not normal and needs your injector immediately.
Post-filler panic is almost always about swelling, you look in the mirror on day two, see puffiness, and assume it 'went wrong.' In the vast majority of cases it's the normal swelling curve doing exactly what it should. Knowing the timeline keeps you from over-worrying, and knowing the danger signs keeps you safe.
The normal timeline
Expect swelling to peak in the first one to three days, then steadily improve over the following week. Lips are the most dramatic, they can look noticeably big and slightly uneven for the first few days, which is normal and not your final result. Cheeks and jawline settle similarly. Most areas look like themselves by two weeks; the under-eye (tear trough) can take up to four weeks and is famous for looking worse before it looks better.

How to ease normal swelling
A cold compress in the first day or two, sleeping slightly elevated, staying hydrated, and avoiding alcohol, salt, and strenuous exercise for a couple of days all help. Arnica may help bruising. Don't massage the area unless your injector specifically instructs you to. Patience is the main treatment, resist judging the result until it has settled.
When swelling is a red flag
Some swelling is expected; some is not. Seek your injector urgently if you have swelling that is severe and one-sided, disproportionate or worsening pain, skin that turns white (blanches) or dusky/blue, or a spreading, hot, tender area with fever. These can signal a vascular problem or infection, both treatable when caught early, which is exactly why you chose a medically trained injector.
